'New Patient' Registration Form
- This form is for patients that have never attended Balmain Sports Medicine previously for any of our services / seen any of our practitioners.
- We will require you to enter all of your Personal and Medical History details, as well as information regarding your Current Injury.
'Returning Patient' Registration Form
- This form is for patients that have attended Balmain Sports Medicine previously for any of our services / seen any of our practitioners, and have booked an appointment for a new injury and / or a new service / practitioner.
- This form will require you only to update any changes to your Personal and Medical History details, as well as information regarding your Current Injury.

Given we offer a range of professional services at Balmain Sports Medicine, there a number of different consultation fees depending on the service / health professional you would like to see. For a list of the relevant costs involved for the service you are enquiring about, please call reception on 9818 1004 and they will advise of the appropriate fees.
For private fee patients a medical referral is required to see one of medical practitioners (sports physician, orthopaedic surgeon, rheumatologist); however it is not required to see our other health practitioners (physiotherapy, pilates, podiatry, massage therapy, dietetics, naturopathy).
Patient’s who are eligible for Compulsory Third Party (CTP) (ie injured in a motor vehicle accident) or Workcover (injured while at work) funding must provide a medical referral (general practitioner, sports physician, orthopaedic surgeon) for treatment, as part of their claims process. However, there are other certain conditions that must be met before bulk billing can be arranged.
We do have a waiting list that our patients can access for their preferred appointments with their current practitioner. To be placed on the list, please speak to reception. For new / acute patients we aim to ensure that you are seen within 1-2 days of your enquiry.
We are open 7am – 8pm Monday to Friday, 8am – 5pm on Saturdays and 9am-5pm Sundays.

If you are catching a bus from Drummoyne (along Victoria Rd): Routes 500 – 502 (Ryde to Drummoyne / Rozelle / City), Routes 504 (Chiswick / Drummoyne / Rozelle / City), Routes 505 (Woolwich / Drummoyne / Rozelle / City) and Routes 506 / 507 (Maquarie University / Ryde / Hunters Hill / Drummoyne / Rozelle / City).

We have limited car parking under the building, accessed via the driveway next to 1 Withecombe St. If you are mobile, we recommend parking in the side streets, Withecombe St or Hancock St, which over 2 hour and 1 hour street parking. Otherwise there is a 2-hour free council car park on Victoria Rd diagonally opposite our practice, entry on Ellen St.
Given that there are a variety of professional health services provided by our practice, there will always be some variations in appointment length, depending on the nature of your injury. However as a general guideline please allow 60min from your scheduled appointment time for your physiotherapy / pilates sessions; other services (podiatry / orthopaedic surgeons / sports physicians) will often be closer to 30 minutes.
Loose comfortable clothing is a general rule. Your practitioner will need to assess your injured body region as part of the assessment / treatment and will often require you to disrobe. Your privacy and comfort is of prime priority, so if you forget to bring the appropriate clothing, we are able to provide you with shorts / patient gowns as needed.
You are able to call the practice directly on 02 9818 1004 or email the practice. If your practitioner is with another patient, we will ensure your message is passed on for them to call you back on your preferred numbers as soon as possible.
Yes we certainly can; if you have private health insurance cover for the professional service seen, please bring your membership card with you, and we will process an immediate rebate for you at the time of your consultation; the amount of this rebate varies depending on the private health fund you belong to and the level of cover you have chosen. You are then required to pay the ‘gap’ (ie the amount of money the private health fund has not covered) using either cash, cheque or credit card (excluding American Express & Diners).
If you see one of our medical practitioners (sports physician, orthopaedic surgeon, rheumatologists), you will be entitled to a Medicare rebate, depending on a set schedule of rebates. In order to claim your rebate, we will provide you with itemised receipts to claim with Medicare.
Our other professional services (physiotherapy, podiatry, pilates, massage, naturopathy) are not directly covered by Medicare; however in certain cases you maybe eligible for certain Medicare programs, such as the Chronic Disease Management program for physiotherapy / allied health. Programs such as these are managed by your General Practitioner (GP), so please discuss these possible options with your GP. If your GP refers you to our practice under one of these program, you will be required to pay our full scheduled consultation fees, then claim back your Medicare rebate with our itemised receipts.
No, we do not process ‘bulk billing’ for Medicare items at our practice, however we do provide itemised receipts for you to process your claim with Medicare (either online or one of their local offices). The nearest Medicare local office is at 23 Balmain Rd LEICHHARDT NSW 2041 (p: 13 20 11).
We have a ’24 hour Cancellation Policy’ to your consultation appointment times. Failure to provide 24 hours notice when changing appointments will result in normal consultation fees being charged. If you are unable to attend your scheduled appointment, please call the practice on 02 9818 1004 and we will, when able, provide you with an alternative appointment time on your scheduled day of service, so as to avoid a cancellation fee being charged.
Our medical practitioners are able to refer you for all medical imaging (XR, ultrasound, MRI, CT, bone scan) which will enable you to receive your full Medicare rebates for each investigation. Physiotherapists and podiatrists are able to refer you for XR and MRI’s at certain radiology practices, that offer ‘non-rebatable’ investigations, which essentially means you are able to access these services at a slightly lower Medicare rebate.
